Your Princeton Club Membership may qualify you for reimbursement from your Health Insurance Company. The companies that offer the reimbursements are: Dean Health Systems, Unity and Group Health Cooperative. Each time you enter the Princeton Club you are required to swipe your card into our computer system. This allows an accurate record of your attendance which is important for you to provide to your health insurance company.

Dean Health Systems
Dean Health Plan's WIN (Wellness Incentives Now!) Program offers cash back for a variety of health-related expenses for the whole family. Anyone who is covered under Dean Health Plan Insurance is eligible to participate, children and adults alike.

Individuals
Get up to $100 back! Dean will reimburse individual members for eligible services received, up to $100 back each contract year.

Families
In addition to the individual maximum, there is also a maximum reimbursement limit of $300 per family, per contract year.

How and When Do I Receive Reimbursement?

1. You pay for the service at the time it is rendered.

2. Send a copy of your membership contract along with the WIN Program Reimbursement Form to Dean Health Plan. For more information, please visit www.deancare.com

Unity
Fitness First Program:
To participate in Fitness First, you need to enroll in the program with Unity before you can begin working toward your chosen reward. If you work out 60x within 6 consecutive months, you and your spouse can earn a combined total of $200 per calendar year in one of three ways:

1. Subscriber earns $200 in two $100 increments: one after 6 consecutive months and the second after another 6 consecutive months.

2. Spouse earns $200 in two $100 increments, one after 6 consecutive months and the second after another 6 consecutive months.

3. Subscriber and spouse split the $200; each earn $100 after 6 consecutive months. For more information please visit: www.unityhealth.com

Group Health Cooperative
Exercise for Excellence:
GHC-SCW fitness reimbursement program. You may receive the wellness reimbursement when 120 or more workouts have been completed during a 1-year period, from January 1st through December 31st. When you have completed 120 workouts, complete the attendance request form, which is available at our front desk. In addition, complete the Wellness Reimbursement Form which can be found at www.ghc-hmo.com. The health club must also sign this form. For more information please visit: www.ghc-hmo.com.

For GHC Members:
The Princeton Club will print attendance requests quarterly on January 1st, April 1st, July 1st and October 1st. If you have filled out the request form during that period, you will have an attendance printout available for pickup at the front desk after the 15th of the above mentioned months. Your printout will include workouts from the previous 12 months. Each time you need a printout you will need to complete a new request form.
